Medigap or Medicare Advantage? Choosing a Plan in New York

Choose Medigap if you want predictable costs and the freedom to see any doctor who accepts Medicare. Choose Medicare Advantage if you want a lower monthly premium and extras like dental and vision, and you don’t mind a network. New York’s rules make the decision less permanent than in most states.

Two different ways to get Medicare

Medigap (Medicare Supplement) sits on top of Original Medicare. Medicare pays first, and your Medigap plan covers some or all of the rest, like deductibles and the 20% coinsurance. You add a separate Part D plan for prescriptions.

Medicare Advantage (Part C) replaces Original Medicare with one private plan. It covers everything Parts A and B cover, usually includes drug coverage, and often adds dental, vision, hearing, and fitness benefits.

Questions to ask yourself

  • Do I see specialists often, or expect to?
  • Do I want to keep a specific doctor or hospital?
  • Do I spend part of the year away from Niagara County?
  • Would I rather pay more each month and less when I get care, or the reverse?
  • How important are dental, vision, and hearing benefits to me?

If most of your answers point to flexibility and predictability, Medigap may fit. If they point to lower premiums and extras, look at Medicare Advantage.

The New York difference

Most states only guarantee Medigap without health questions in a one-time window at 65. New York requires insurers to accept you year-round, and premiums can’t be based on your age or health. If you start with Medicare Advantage and change your mind, you can move to Medigap later.

The tradeoff is that Medigap premiums in New York tend to run higher at 65 than in other states, because everyone in an area pays the same rate.

Frequently asked questions

Is Medigap better than Medicare Advantage? Neither is better for everyone. Medigap offers more predictable costs and doctor choice. Medicare Advantage offers lower premiums and extra benefits.

Can I switch from Medicare Advantage to Medigap in New York? Yes. New York lets you apply for Medigap any time of year without health questions. You’d return to Original Medicare and add a Part D plan.

Do Medigap plans cover dental and vision? No. Medigap plans cover Medicare cost-sharing, not dental, vision, or hearing. Some people buy separate dental or vision coverage.
