Medicare 2027 Changes: What Niagara County Should Know

The biggest Medicare changes for 2027 are in prescription drug coverage. The yearly Part D out-of-pocket cap rises to $2,400, and the maximum Part D deductible rises to $700. At the same time, lower negotiated prices take effect for 15 widely used drugs. Here’s what that means for your wallet.

Your drug costs are capped at $2,400

Part D now has a ceiling on what you pay each year for covered drugs. In 2026 it’s $2,100. On January 1, 2027, it becomes $2,400. Once you reach it, your plan covers your drugs at no cost for the rest of the year. The cap applies to standalone Part D plans and Medicare Advantage plans with drug coverage.

Deductibles can go as high as $700

Plans can’t charge a Part D deductible above $700 in 2027, up from $615. Many plans charge less or nothing. Your plan’s Annual Notice of Change shows its actual deductible.

15 drugs get lower Medicare prices

Medicare negotiated prices for 15 more drugs, and those prices take effect January 1, 2027:

UseDrugs
Diabetes and weightOzempic, Rybelsus, Wegovy, Janumet, Tradjenta
LungsTrelegy Ellipta, Breo Ellipta, Ofev
CancerXtandi, Ibrance, Pomalyst, Calquence
OtherLinzess, Xifaxan, Otezla, Austedo, Vraylar

CMS expects these prices to save people with Part D about $685 million in out-of-pocket costs in 2027. Your savings depend on your plan.

Some premiums may rise

A temporary federal subsidy that kept standalone Part D premiums down ends this year. Expect some plans to raise premiums for 2027. Compare before you renew.

Medicare Advantage plans are shifting too

Some Medicare Advantage plans are trimming extra benefits like dental, raising co-pays, or leaving certain counties. If your plan leaves Niagara County, you’ll get a letter and a Special Enrollment Period to choose a new one.

Still waiting: 2027 Part B costs

Medicare announces the next year’s Part B premium and deductible in the fall. In 2026, the premium is $202.90 a month and the deductible is $283. We’ll update this post when 2027 numbers are out.

What isn’t changing

  • Covered insulin still costs no more than $35 a month under Part D.
  • You can still spread drug costs across the year with the Medicare Prescription Payment Plan.
  • New Yorkers can still apply for a Medigap plan any time of year.
